(1) VALID PASSPORT six (6) months from the date of travel, including old passport/s
-Passport with at least two blank and unused visa page
-Application may not be accepted if passport is broken, damaged or soiled.

(2) COMPLETED visa application form - should be printed in A4 size paper | DOWNLOAD FILE ->
-Please do not staple documents. Issuance receipts of certificates are not necessary and to be removed.
-Application forms with blank fields will not be accepted. If you don’t have applicable information, please fill in “N/A”
-Application forms without date of application and signature will not be accepted. For the date of application, please fill in the date of submission to the accredited travel agency

(3) Two (2) pcs recent photo 
-Please submit a 4.5 x 3.5 (cm) photo taken within six months from the date of application.
-Please write your full name and date of birth on the back of photo.
-Colored photo on White background
-Applicant should be in decent attire: dark top with collar & sleeves

☞ If there is a used Japanese visa on your passport, the following (4) and (5) are unnecessary.

(4) ORIGINAL Philippine Statistics Authority (PSA) issued & authenticated birth certificate, within one (1) year from the time of filing

• For applicants without previous and used Japan visa 

• For MARRIED only

Important Notes:

1. For PSA Birth Certificate (born from 1940 to present) under "Late Registration", applicants are required to submit their ORIGINAL/PHOTOCOPY BAPTISMAL CERTIFICATE AND SCHOOL RECORD FORM-137  (Gradeschool or Highschool only)  

2. For PSA Birth Certificate with "Unreadable Details", and if "No Record Found", applicants are required to submit a CERTIFIED TRUE COPY OF THE BIRTH CERTIFICATE ISSUED BY LOCAL CIVIL REGISTRY or CERTIFICATION ISSUED BY THE NATIONAL ARCHIVES

(5) If MARRIED, ORIGINAL Philippine Statistics Authority (PSA) issued & authenticated marriage contract, within one (1) year from the time of filing

• For applicants without previous and used Japan visa

Important Notes:

1. If there is "No Record of Marriange" in PSA, , applicants are required to submit a CERTIFIED TRUE COPY OF THE MARRIAGE CERTIFICATE ISSUED BY LOCAL CIVIL REGISTRY and NEGATIVE CERTIFICATE issued by the PSA

2. For PSA Birth Certificate with "Unreadable Details", applicants are required to submit a CERTIFIED TRUE COPY OF THE MARRIAGE CERTIFICATE ISSUED BY LOCAL CIVIL REGISTRY 

(6) Daily Schedule / Itinerary in Japan

【In case that applicant will shoulder part/all of travel expense】

A. ORIGINAL Bank Certificate
The following should be seen in the bank certification: 
(1) Type of account - Savings or Current
(2) Date Account was opened
(3) Account Number
(4) Average daily balance (ADB) for the last 3 months

B. Copy of Income Tax Return (Form 2316)
Must have a GROSS INCOME and STAMP OF RECIPIENT, if tax is not withheld (e.g. self-employed)

If (B) Copy of ITR is not available and there is no ADB (Average Daily Balance) on (A) Bank Certificate, submit a Bank Statement to prove transaction for the last 3 months

【In case that guarantor in the Philippines will shoulder part/all of travel expense】

※ Guarantee Letter
※ Proof of relationship between applicant and guarantor(e.g. Birth Certificates)
※ Bank Certificate
※ Photocopy of Income Tax Return (BIR: Form 2316)

For EMPLOYEE: Certificate of Employment stating position, tenure of service and with gross annual income 

For SELF EMPLOYED: DTI Business Name Registration and/or SEC Registration Certificate

1. For GOVERNMENT EMPLOYEE, approved leave of absence is required

ADDITIONAL NOTES:

1. For FOREIGN PASSPORT HOLDER, I-CARD Copy is required

2. For SPONSORED APPLICANT, Guarantee letter is required
